Care Coordination is the new name for case management. The IDoA has gone through a series of changes over the past several years and this is one that was made as a result of changing the assessment tool used to gather information and determine eligibility of clients for services. IDoA also change the title of Case Manager to Care Coordinator. These changes are in line with what is happening all across the US with health care reform as agencies are looking at more effective ways of providing holistic care to clients regardless of their age. Care Coordinators are required to assess cognitive skills, environment, health, formal and informal support, etc to create a plan of care that will allow the client to maintain his/her independence in a community environment.
