CDFSS provides an array of client services that case managers must access in order to enable older citizens of Chicago to live independently in their homes and communities for as long as possible. The case manager facilitates the access to ensure the delivery of integrated services to all clients by providing a timely review and assessment of the client’s condition and services received.
After a home visit is completed, the service plan is developed and approved; referrals for service are initiated once case management informs the providers with the necessary information/documentation needed to begin service. The case managers link clients to both purchased service and non-purchase of service and other resources.
