Healthcare Consortium of Illinois is contracted with both the Illinois Department on Aging and the Chicago Department of Family & Support Services to provide care coordination and case advocacy to seniors 60 years and older in the City of Chicago and South Suburbs. Programs included in the department are: Care Coordination (CC) Elder Abuse & Neglect (EAN) Veterans Independence Program (VIP) Money Follows the Person (MFP) Senior Community Service Employment Program (SCSEP) Office of Rehabilitation Services (ORS), and Unity challenge.
