Consortium of Illinois, MTA 14 HIE Project team has developed a survey
designed to assess MTA 14 stakeholder and provider capacity. This survey
is your opportunity to provide information about your readiness to
participate in the Health Information Exchange being developed to
achieve meaningful use in Medical Trading Area 14. Your answers will
