The Office of
Community Health serves to lead and
intensify efforts that promote and develop the optimal health of the
communities its services by serving as an advocate for the allocation of
resources and the development of policies. Several collaborative efforts
that address community health related issues are housed within this
department.
Some efforts include:
Healthy Start Southeast Chicago, an infant mortality reduction
initiative targeting six communities of Chicago South Side, focuses on the
need to strengthen and enhance community systems of maternal and child
health care, as well as challenges communities to fully address the medical,
behavioral and psycho-social needs of women and children.

Southeast Chicago Diabetes Action Coalition addresses the devastating
effects of the chronic illness, diabetes. Targeting six communities on the
city’s southeast side, the project aims to reduce diabetes mortality and
associated complications and disabilities among residents, to delay the
onset of diabetes among those at risk for developing diabetes and to promote
the highest quality of life among African Americans and Latinos who reside
in the area.

The Breast Cancer Program is a comprehensive directory network to
provide educational resources and information to the under serviced women of
the Greater Roseland area.

The Obesity Program works with a
number of other organizations to promote awareness on obesities and ways
that it can be prevented.

The
Faith Based Initiative is one of HCI’s newest programs which focuses on
increasing communication, coordination, and collaboration between the
healthcare providers, ministries, and citizens of the Greater Roseland
community.
